On behalf of Physicians Choice, we would like to take a moment to provide some clarification regarding
claims submission and claims payments.
Do not send claims to Managed Care Systems.
All Professional claims regardless of the date of services, should be sent to Physicians Choice Medical
Group, care of Identity MSO as follows:

Claims are processed by Identity MSO. If you have questions regarding claims please call 1 (888) 560-
8799.
Payments are processed by ECHO. If you do not want to receive payments via the virtual payment card
(which will cost you 1.9% of the payment), you may opt out and request paper checks. To opt out please
call ECHO directly at 1 (440) 835-3511, choose option 3.
